Dr. Damon Cormier is a native of Jennings, Louisiana. He studied a pre-chiropractic curriculum at McNeese State University and Bernau
College of Atlanta, Georgia. He attended Life Chiropractic College of Marietta, Georgia and attained his Doctor of Chiropractic degree
graduating Magna Cum Laude in 1992. Dr. Cormier also completed 120 hours of Physiological Therapeutics from National College of Chiropractic
in 1991.
Dr. Cormier has been licensed to practice in the states of Louisiana and Texas since 1992. His experience includes a chiropractic
internship at Life Chiropractic College in Marietta, Georgia. He also tested into the Life Chiropractic College Senior Intern Program.
Out of 3,000 students approximately 20 interns were able to test into this program per quarter. As a Senior Intern, Dr. Cormier was
responsible for supervising and assisting his fellow chiropractic interns as they treated their patients. This program required that
he be knowledgeable and proficient in multiple Chiropractic techniques as well as radiology and advanced diagnostics. Because of his
proven level of proficiency, this program also allowed him the privilege of treating his own patients during this time unsupervised.
He is certified in Gonstead technique, x-ray and case management by the Life Chiropractic College Gonstead Study Club. He has been a
member of the American Chiropractic Association, the Chiropractic Association of Louisiana, and the International Chiropractic Pediatric
Association. He has served on the Board of Directors for the Chiropractic Association of Louisiana. He is a member of the Pi Tau Delta
Honor Fraternity for superior scholastic achievement and a member of the International Chiropractic Honor Society.
Dr. Cormier has practiced Chiropractic in Lake Charles, Louisiana since 1992. He lives there with his wife, Tess and they have three
children, Jeanne, Caleb, and Sarah. He is an active member of Our Lady Queen of Heaven Parish and participates in numerous Our Lady
Queen of Heaven parish and school functions. He and his wife are members of the Krewe de la Famille, a krewe that encourages family
activities throughout the year. Dr. Cormier also enjoys activities such as hunting, fishing, cooking with his family, and spending
time with family and friends.

Dr. Scott P. DeRouen received his Doctor of Chiropractic (D.C.) degree from Life University in 1995. After graduating he began his
private practice with Nature’s Way Chiropractic Center and was with Nature’s Way until December, 2009. In December, 2009, Dr. DeRouen
joined the Center for Chiropractic and Rehabilitation where he is currently a practitioner. His practice is focused on family, sports,
and accident and injury.
While a student at Life University, he focused on additional training in the art, science and philosophy of chiropractic. Dr. Scott
DeRouen has nearly four years of advanced training in the Gonstead method. The Gonstead system is one of the most comprehensive methods
in the chiropractic profession. The technique focuses on disc restoration and leveling the vertebrae. The Gonstead system utilizes
instrumentation, specific manual (by hand) adjustments, and adjusting the extremities (limbs). The scope of the technique ranges
from pediatrics to geriatrics. As a graduate student Dr. Scott earned a certification from the Gonstead Study Club at Life University.
In addition, he has completed over 225 study hours of advanced training with the Gonstead Clinical Studies Society (G.C.S.S.)
in Wisconsin. He is also eligible for diplomatic testing in the Gonstead technique. Dr. Scott has spent many hours instructing hundreds
of chiropractic students in the Gonstead method at Life University and Parker Chiropractic College.
Dr. Scott received his Bachelor of Science Degree in psychology, with minors in sociology and chemistry, from Lamar University in
Beaumont, TX. As a undergraduate, he was involved in Leadership Lamar and Sigma Nu Fraternity. He was also highly involved in martial
arts since the age of 15. He holds two black belts, one in Universal Karate (mixed martial arts) and the second in the art of Aikido
(Tomiki Ryu). In Universal Karate he holds top honors of Master Level (2nd Degree). In Akido he holds a first degree. He also had a
passion for endurance training. He competed in 3 ½ Ironman Triathlons and 4 century races (100 miles) on a bike.
As a youth, Dr. Scott was involved in baseball, golf team, football (Jr. High and High school) and cross country running.
Dr. DeRouen is married to Kim Clarice Manning-DeRouen and they have a daughter Joli DeRouen. His hobbies include studying Theology,
bow hunting and spending time with his wife and child.

Dr. Donald R. Thigpen is a native of Westlake, Louisiana, and is a graduate of McNeese State University, the Louisiana Institute of Massage
Therapy, and Texas Chiropractic College in Houston, Texas, totaling eight years in institutions of higher learning. He speicalizes
in spinal rehabilitation and is a Certified Spinal Decompression Practitioner. In addition to neck and back rehabilitation,
he commonly treats aliments of the extremities such as shoulders, elbows, wrists, hips, knees and ankles.
Dr. Thigpen received his Doctor of Chiropractic degree from Texas Chiropractic College and was valedictorian of his graduating class.
While at Texas Chiropractic College, he received an extensive education in human anatomy, physiology, pathology, and other clinical
and biological sciences. In May of 2001, under the direction of Dr. Bill McGimsey, Ph.D., Dr. Thigpen was appointed to the position of gross
anatomy lab instructor and assisted students in the dissection of human cadavers. He retained this position for 2 years performing dissections
on over 150 human cadavers. In January of 2002, under the direction of Dr. Hasnaa Shafik, M.D., Ph.D, Dr. Thigpen was appointed to the
position of histopathology lab instructor where he assisted students in the microscopic identification of human tissues and diseases.
Dr. Thigpen's experience includes a chiropractic internship along with rotations in neurosurgery and rheumatology. His rotation in
neurosurgery was conducted under the supervision of Dr. Edward Murphy, M.D., at St. Joseph's Hospital in Houston, Texas. During his
rotation in neurosurgery, Dr. Thigpen assisted Dr. Murphy with neurological examinations, observed and assisted in surgery, and received
extensive instruction on neurological conditions, brain MRIs, and spine MRIs. His rotation in rheumatology was conducted under the supervision
of Dr. Martin Lidsky, M.D., at the Veteran's Affair's Medical Center in Houston, Texas. Under the instruction of Dr. Lidsky, Dr. Thigpen
gained valuable knowledge and experience on the proper evaluation and diagnosis of many arthritic and rheumatic conditions.
Prior to attending Texas Chiropractic College in Houston, Texas, Dr. Thigpen completed his undergraduate studies at McNeese State University
in Lake Charles, Louisiana. In May of 2000, he graduated cum laude and received a Bachelor of Science degree in pre-medical biological
sciences. While at MSU, because of his academic standing, he was selected by MSU faculty to participate in the Senior Honors Thesis
research project, which he completed under the direction of Dr. Ray Neyland, Ph.D. As a graduate of the Louisiana Institute of Massage
Therapy, Dr. Thigpen was valedictorian of the class of August 2000. He was a nationally certified and licensed massage therapist in the
state of Texas from 2001 until 2004. Currently, Dr. Thigpen practices chiropractic medicine in Lake Charles, LA. Also, as a former instructor
for the Louisiana Institute of Massage Therapy, he taught human anatomy, physiology, and pathology.
Dr. Thigpen is also an Elsevier reviewer and has been recognized in three text books for his contributions. You can find him listed as a
contributing reviewer in Mosby's Pathology for Massage Therapists 2nd Edition, Massage Therapy Principles and Practice 3rd Edition and
Massage Therapy Principles and Practice 4th Edition.
Dr. Thigpen is a member of the Chiropractic Association of Louisiana, American Chiropractic Association, Louisiana Chiropractic Political
Action Committee, Federation of Chiropractic Education and Research, Better Business Bureau, National Federation for Independent Business,
Chamber of Commerce and Fusion Five.

Dr. Kulaga grew up in DeRidder, Lousiana and is a DeRidder High School graduate. He graduated from McNeese State University in
Health and Human Performance. Upon graduation from MSU, Dr. Kulaga enrolled at Texas Chiropractic College to pursue his Doctor
of Chiropractic Degree. After completing a rigorous post graduate studies program and completing an internship in an outpatient
setting at Moody Health Clinic, Dr. Kulaga received his Doctor of Chiropractic Degree in December of 2008 and passed all
parts of the National Board of Chiropractic Examiners. After passing his Louisiana State Licensure to practice Chiropractic,
he began his career in Lake Charles. Dr. Kulaga has a passion to help people by reaching out to the community and educating
his patients about the importance of spinal and extremity preventative health and physical medicine. Dr. Kulaga has been in
practice for nearly two years before joining with Center for Chiropractic and Rehabilitation. He is a Certified Chiropractic
Sports Physician (C.C.S.P.) and is also certified in KenesioTape. These advanced skills allow him to better assist his patients
in sports repetitive and overuse injuries and also give him more tools to better serve and help his patients to maximize their
recovery from pain and injury. Dr. Kulaga believes in helping people one patient at a time to help them overcome the obstacles
and stress of life.
Dr. Kulaga has been married to his beautiful wife, Brandi Kulaga, for almost six years. He is an avid workout enthusiast who
lives a healthy lifestyle. He also enjoys to fish on the weekends. When Dr. Kulaga is not spending time with his wife and family,
he is pushing iron, fishing on Big Lake, actively involved in his church, or telling people how Chiropractic care can help better
their way of life.

Dr. Baynard was raised in Moss Bluff and graduated from Sam Houston High School. She attended McNeese State University graduating with a
degree in Health & Human Performance with a minor in Chemistry. Following McNeese, she went on to earn her Doctorate of Chiropractic
from Texas Chiropractic College while also obtaining a second Bachelor’s degree in Human Biology. At the end of her chiropractic education,
she completed an internship at the Moody Health Center and was selected to be chief intern within the first three months of her internship.
She also completed hospital rotations in Radiology, Internal Medicine, Pediatrics, Orthopedics, and was involved in two chiropractic
preceptorships prior to graduation. Dr. Baynard was also selected to participate in a Sports Medicine rotation at Rice University in Houston.
She worked on their athletes (mainly Football and Volleyball) implementing techniques such as the Graston Technique, Myofascial Release,
Kinesiotape, and of course, chiropractic manipulation.
Dr. Baynard is also a certified personal trainer with the Cooper Institute of Dallas. She has taken extra courses in Pregnancy &
Exercise from the Cooper Institute and completed coursework in the Graston Technique at Texas Chiropractic College. She is currently
taking classes to become a Certified Chiropractic Sports Physician (C.C.S.P.).

Michael A. Breaux, LMT, has been involved in massage therapy since 1987, and is a graduate of Susan Salvo's original
workshop program from the Louisiana Institute of Massage Therapy. He has studied many styles of massage, but specializes
in clinical massage, sports massage, and trigger point work. He has practiced massage therapy in hospital, physical
therapy, and chiropractic settings in addition to his private practice. His clients include olympic athletes and
members of the Ladies Pro Golf Association.
Mr. Breaux is the Co-Director for the Louisiana Institute of Massage Therapy in Lake Charles, LA. He is also a
contributing author for the text book Massage Therapy: Principles and Practice, having written the clinical massage
chapter.
Mr. Breaux is a member of the International Massage Association. In 1998 he was appointed by Governor Mike Foster
to the Louisiana State Board of Massage Therapy. Mr. Breaux served for four years on the Board, three as Board Chairman,
and one as hearing officer.

Debbie Dayton, LMT, MMP, is a native of Eunice, LA, and is a 2005 graduate of Blue Cliff College. She has studied
many styles of massage therapy, but specializes in medical massage, myofascial release, deep tissue and hot stones.
She has practiced massage therapy in both spa and clinical settings. Her clients have included patients with fibromyalgia,
neck and back pain, extremity pain, carpal tunnel syndrome, headaches, plantar fasciitis, and scoliosis.
Mrs. Dayton has extended training in clinical massage. She is a certifed Medical Massage Practitioner and has completed
extended training courses in deep tissue massage and myofascial release. She is also a nationally certified massage therapist.

Missy Richard,LMT, is a native of Ville Platte, LA. She attended McNeese State University graduating with a degree of
Associate of General Studies in 2002. She also graduated from Louisiana Institute of Massage Therapy in Lake Charles, LA.
Some of the massage styles she studied are: sports massage, trigger point, and myofascial release. She has practiced massage
in clinical settings, spa settings and in her private practice.
